Patients with chronic myeloid leukemia in chronic phase have a life expectancy similar to general population in case of tyrosine kianse inhibitor treatment. Despite these improvements, the major goal for hematologists is to determine the optimal treatment approach for rare advanced CML phases (primary or therapy induced and associated with clonal evolution). Advanced disease consists of mixed clinical manifestation and also brings some unusual and often life-threatening situations. Our case report presents a rare picture of disease progression during tyrosine kinase inhibitor treatment with severe hemorrhagic manifestation in blast crisis, posttransplant relapse and also a review of published literature.
