Isolated Kidney Infiltration by Suspected Leukemic Cells – an Unusual Case Report with Complex Differential Diagnostic

This case report characterizes a serious suspicion of isolated diffuse kidney infiltration by myeloid leukemic cells, however, without primary bone marrow involvement and any pathological imaging finding, in a patient who suffered from a fever of unknown origin, acute kidney injury, microcytic anemia, weight loss, anorexia, dry cough, and mild splenomegaly. A complex diagnostic process of unusual and for hematologist inexplicable case report required repeated biopsy examinations of both bone marrow and the kidney, which subsequently ruled out leukemic infiltration. Symptomatology and pathological laboratory findings responded promptly to corticosteroid treatment, and our patient's condition was finally diagnosed as tubulointerstitial nephritis.
